About the Hymen

The hymen is a thin membrane of connective tissue over the entrance of the vagina into the vestibule. It has a central opening that is usually round. Thin mucosal septae may create two or more openings, or the hymen may be cribiform, having numerous small openings through a solid mucosal plate. On rare occasions, it may be imperforate so that no communication between the vagina and the vestibule is present The hymen varies somewhat in thickness and elasticity. It may be quite elastic and easily stretched without laceration at initial sexual intercourse, or it may be so tough and rigid as to prevent intercourse. As a rule, an intact hymen is present only in a virgin; after coitus, it is represented by small, membranous elevations called the carunculae myrti-formes.

The hymen is lined on both sides by a thin mucous membrane composed of stratified squamous epithelium Mahran and Saleh (1965) examined the hymen microscopically at different stages of life from birth to menopause. They found that the main bulk of the hymen is formed of fibrous connective tissue, partly elastic and partly collagenous fibers. The lining squamous epithelium covering both surfaces lacks any evidence of cornification. No glandular or muscle elements are present. The hymen is not richly supplied with nerve fibers. No nerve cells or fibers are present at the free edge of the hymen. Elsewhere in the structure, the terminal nerve fibers penetrate the epithelium between the connective tissue papillae. In the newborn, the hymen is vascular, and the epithelium is thick. During pregnancy, the epithelium is very thick and very rich in glycogen. At menopause, the epithelium is thin.

Childbirth often causes vaginal relaxation, where the muscles become relaxed and have poor tone, strength, and control. The internal and external diameters of the vagina increase. Thirty million American women suffer from symptoms of vaginal relaxation and stress urinary incontinence. Many women have difficulty controlling their urine in certain situations or notice changes in their bowel habits. These two symptoms may be related to a common set of problems that may occur as a result of childbirth, aging or a combination of both. Grouped together these problems are referred to as pelvic relaxation.

The goal here is to acquaint you with the various forms of pelvic relaxation as well as their causes, symptoms, and treatment. The pelvic organs include the vagina, uterus, bladder, and rectum.

These organs are held in position by three types of supports:

1. muscles
2. sheets of tissue called fascia
3. ligaments

When these supports become damaged for various reasons, one or more of the pelvic organs may sag and, occasionally, even protrude outside the vagina. These are called pelvic support defects.

During childbirth, as the baby passes through the birth canal, the muscles, fascia, and ligaments separate and may be weakened. This weakening gradually worsens and, in later years, may cause the pelvic organs to drop from their normal positions.

Occasionally, this weakening of the muscles and tissue may occur in women who have never had children. In these women, the cause may be:

* Inherited weakness of the supporting tissues
* Unusual strain placed on the supporting tissues by a chronic cough
* Unusual increases in abdominal pressure
* Obesity

What are the symptoms of Pelvic Relaxation?

The general symptoms associated with pelvic relaxation depend on which organs are affected. Often there is a feeling of heaviness or fullness. Small or moderate amounts of urine may be lost with normal physical activities such as laughing, coughing, walking, or running (read about Stress Urinary Incontinence). In more advanced and rare cases a mass may actually protrude from the vaginal opening. Based on the organ or organs involved, pelvic support defects can be defined more specifically as:

* Cystocele
* Urethrocele (most of the time the cystocele and urethrocele occur in combination - cystourethrocele)
* Rectocele
* Enterocele
* Uterine prolapse

A cystocele occurs when the bladder falls or descends from its normal position. The most common symptom associated with cystocele is difficulty in completely emptying the bladder. This can be associated with bladder infections. Large cystoceles can cause the bladder to overfill and allow small amounts of urine to leak. Leakage is most common during activity such as walking or bouts of coughing.

A urethrocele usually occurs in conjunction with a cystocele. Both of these conditions result in, among other things, involuntary loss of urine, particularly when there is increased pressure in the abdomen, caused by walking, jumping, coughing, sneezing, laughing, or sudden movements. Rectoceles happen when the rectum bulges into or out of the vagina.

Rectoceles usually occur as a result of injuries sustained during childbirth. With a weakened or bulging rectum, bowel movements become more difficult.

An enterocele is the bulging of small intestines into the back wall of the vagina.

Uterine prolapse occurs when the uterus falls or is displaced from its normal position. There are varying degrees of severity depending on the descent. This produces a general felling of heaviness and fullness, or a sense that the uterus is falling out.

The diagnosis of these problems includes a through history and physical examination. Other tests, depending on the circumstances, include a "Q-tip" test, urodynamic studies (a painless fifteen to twenty minute computerized bladder and urethra functional studies), urethrocystoscopy (instrument used to evaluate the inside of the bladder and urethra), X-rays of the urinary system.
For all practical purposes, definitive treatment is surgical correction of the specific defects.

Hymenoplasty or restoration of ruptured hymen was one secret most Pakistanis knew was practised clandestinely. They’d whisper the addresses of doctors performing the surgery to harried girls keen to reclaim their virginity weeks before their nuptial night. Call it the irony or hypocrisy of our times, but even as Islamists seek to impose their worldview on a schizophrenic society, hymenoplasty is witnessing a boom. Doctors now advertise in English newspapers, on the internet and walls of shops on busy street corners.

All this is good news for girls who have had premarital sex but for some reason didn’t marry their partners, as it is for those who tore their hymen accidentally. Persuaded or compelled into betrothal, they dread the wedding night. In a society where premarital sex is a religious taboo and family honour is perceived to lie in the daughter-in-law’s intact hymen, many a women have been divorced instantly or paid with their lives for not bleeding on the wedding night.

Classified as cosmetic surgery, hymenoplasty was exported here from the UK, where the surgery became popular among Pakistani girls who experimented with sex but found, to their horror, parents marrying them to orthodox boys back home. Re-virgination began in Karachi, came to Islamabad and has become quite a rage in Lahore. Performed at $500, or Pakistani Rs 40,000, hymenoplasty is usually resorted to by sexually-liberated upper class girls for whom the price of surgery is too small for a lifetime of marital tranquility.
Nor is the hymenoplasty procedure cumbersome. Performed under local anaesthesia, the surgery could last anywhere between 30 minutes and two hours—and requires the person undergoing it to abstain from sex for three months. Developed first by Toronto-based plastic surgeon Dr Robert H. Stubbs, the procedure requires suturing of a tear in the hymen, or using tissue from the vaginal wall to restore it. Once the person with a mended hymen has sex, she experiences pain and, more significantly, bleeds, hoodwinking those Pakistani men who see in the stained bedsheet proof of their honour.

But the sharp spurt in hymenoplasty is a story more about Pakistan’s sexual liberation than doctors coming out in the open—and the society accepting it. This is what I realised as I sought to gather details about it. My first port of call was Dr Farooq Nasim, who owns the Nasim Fertility Clinic in the middle-class locality of Johar Town in Lahore. As my photographer-friend turned his camera on Dr Nasim, he became apoplectic, curtly telling us that he hadn’t even heard of hymenoplasty. Next, I talked to him over the phone, posing as a worried partner of a girl wanting to become a virgin again. Dr Nasim intoned: “I haven’t heard of hymenoplasty.”

I consequently requested journalist-friend Nida (name changed) to help me breach Dr Nasim’s wall of caution. She dialled the mobile number advertised in newspapers; it was of Dr Nasim’s assistant, from whom she sought an appointment for hymenoplasty. She was provided a token number that she was to spell out to the guard at the gate. And the gate promptly opened on D-day. Nida was ushered into Dr Nasim’s chamber. The doctor’s reticence evanesced at the sight of a prospective customer. “We charge only Rs 40,000; abroad, the operation costs $2,000 or more,” he told Nida. “Since ours is a conservative society, we don’t ask girls to register their names.” Such benevolence helps pull in customers: Dr Nasim claims to have restored 300 hymens in the last two years alone.
Perhaps surprised to see Nida alone, the doctor said, “In most cases, girls come with a female friend or their partner. Since the procedure doesn’t take more than a couple of hours, it’s easy for customers to conceal the procedure.” He then switched to playing pop sociologist: “Today’s youth have gone beyond their parents’ expectations (read morals).” And so have the doctors, you could say. Next, I turned to the internet. www.hopepk.com, for instance, offers hymenoplasty, provides a mobile number (0092-323-4195732) and an electronic appointment form. The address of the hospital where the operation is to be performed is divulged when the customer is given a date. The website belongs to a Lahore-based couple, Dr Sarfaraz Ahmed and Dr Yasmin Sarfaraz. Gynaecologist by training, it’s Yasmin who performs hymenoplasty. During her telephonic conversation with Nida, Yasmin offered to perform both hymenoplasty and labiaplasty, which is a procedure to tighten vaginal muscles by reducing excess vaginal lining. “The result,” the couple’s website says, “is an immediate decrease in the size of vaginal muscles, resulting in more friction during intercourse.” Labiaplasty is catching up among Pakistani upper classes Occasionally, doctors performing hymenoplasty resort to fake identities. For instance, Dr Syed Rizwanul Haq runs www.noorclinic.com, which, among other things, offers e-books on sex authored by one Dr Arshad Javed. Ask Dr Haq for Dr Javed’s contacts and you draw a blank. The reason: Dr Haq and Dr Javed are the same person.

Dr Javed asked Nida to meet him at the Bio-Test Clinic, 681-Shadman I, Lahore. After undergoing stringent security and identity checks, she was ushered into his office. He was candid, “We have to be cautious because ours is a conservative society. I don’t think the maulvis have any inkling about this phenomenon, otherwise they would have kicked up a ruckus!” As Nida furrowed her forehead, the doctor gratuitously clarified, “The girls coming to me include those who had injured themselves during sports. I’m helping them. I’m not doing anything illegal.” (He said he had performed hymenoplasty on 100 girls in the last two years; the number relatively less because he advertises only on the website.)

The Pakistan Medical Association was unwilling to speak to Amir Mir on hymenoplasty. But its senior member spoke on condition of anonymity, “Whatever the legal position, I won’t condemn it as it is for someone’s good.” And, obviously, good for the sexually liberated who feel stifled in a restrictive society. Their numbers, as the booming hymenoplasty business shows, is high—and growing.

Women are having surgery to rejuvenate their love lives

(February 27, 2006) - When Jeanette Yarborough decided to give her husband a gift for their seventeenth wedding anniversary she wanted it to be special. Really special. She decided that conventional treats such as Mediterranean cruises, gold watches, cars, a murder-mystery weekend, or even a boob job just weren’t going to cut it. She gave him something much more personal — and painful. Her virginity.

Well, sort of. Mrs. Yarborough paid $5,000 (£2,860) to a cosmetic surgeon to stitch her hymen back together so she could “lose her virginity” all over again and her husband would have that thrilling conquest at the grand age of 40.

He did, and after that very expensive moment the ecstatic couple spent a passionate Valentine’s weekend last year having the kind of sex that they had almost forgotten about. Now they are busy telling family, friends and strangers that it is the best money they ever spent and everyone should do it.

“Now my sister is thinking of becoming a virgin again for her 45th birthday to surprise her husband,” says Mrs. Yarborough gleefully, as she sits in her modest family home in San Antonio, Texas, talking unabashedly about such intimate matters.

She is not the first to choose the operation — a hymenoplasty — to repair the fragments of skin forming the traditional “gateway” to the vagina, years after originally losing it.

Women have resorted to backstreet hymen repair for centuries in religions and cultures in which marrying as a virgin is sacred and losing your “maidenhead” before matrimony can mean shame, or even being put to death. But an increasing number of women such as Mrs. Yarborough are now electing to be “revirginised” using modern techniques as a purely cosmetic or lifestyle choice, to “put the sparkle” back into their marriage or give their husband a surprise on the second honeymoon.

They usually opt also to have one of the new “designer vagina” procedures, such as tightening up of the vaginal canal slackened by childbirth, or the cosmetic trimming of enlarged labia.

“I have affluent upper-class ladies coming in from Manhattan, getting ready for a second-honeymoon cruise or something like that. Or some women had a disappointing time the first time they were deflowered and now they have found someone special they would really like to give it up to,” says Dr Marco Pelosi, a gynaecologist and plastic surgeon who has a specialist clinic in Bayonne, New Jersey. He performs ten hymenoplasties a month.

“Ninety per cent of them are for women who are in big trouble if they do not appear to be a virgin when they get married. Then there are the small number who just want it done,” he says. For six to 12 weeks after the operation the woman cannot have sex or exercise vigorously while she heals up. Then she is ready to return, in a flash of additional pain, to her deflowered state. “Thousands of dollars, and it lasts a few seconds. People think it’s crazy but to my patients it does not matter. It means such a lot to them,” says Pelosi.

The operation is performed under general anaesthetic and takes up to an hour. The fragments of the broken hymen are made raw again using a laser, which cuts and cauterises simultaneously. Then the fragments are pulled together and stitched, leaving only the small vaginal opening associated with virginity.

The technique was pioneered by Dr David Matlock, a Beverly Hills plastic surgeon, about five years ago and he has trained a handful of others, most of whom have fanned out across the US, with one or two in Canada, to set up their own clinics (there are only about a dozen cosmetic surgeons in the US offering the procedure).

One doctor in Connecticut markets extensively in magazines and on the internet to British clients, offering international vaginal makeover packages that include flight, limousine transfer, hotel — and hymenoplasty. Most clients are Latin Americans, Saudi brides-to-be or British Muslims who fly in to be surreptitiously revirginised before marriage. But there is also a growing demand for “recreational” hymenoplasty. Indeed, it ’s now so common at two New York clinics that the price has dropped to $1,800 (£1,029).

Named after Hymen, the Greek god of marriage, the vaginal membrane has been a marker of virginity since the Stone Age, even though it can be ruptured by nonsexual activity, such as athletics or wearing tampons. It has always been a sensitive topic: Dr Matlock told The Times that he was happy to talk about all the “designer vagina” operations he offers — except hymen repair, because he has had death threats from religious groups outraged that the fallen faithful can buy a fake virginity. Even the American plastic surgery industry, which convinced nine million people last year — up by a quarter since 2000 — that their lives would be better if only they remodelled their breasts, thighs, tummy, bottom, eyelids, cheek, nose, etc, draws the line at “revirgination”.

Hymenoplasty is not licensed by any official plastic surgery or gynaecological association, it is not officially taught and it is so new and on the fringe that there are only anecdotal statistics. All the operations are done privately and paid for in full by the individual.

None of which bothers Jeanette Yarborough, who decided to have her hymen reconstructed in a combination operation with vaginal “rejuvenation” tightening. She looks adoringly at her husband, Louis, as she says: “What an awesome gift to give the man in my life who deserves everything. It was the most amazing thing I could give him as a woman,” she says. Louis Yarborough, 44, explains hastily that he did not expect such an extreme gesture from his wife and tried to dissuade her from going through the painful revirgination experience — both the surgery and losing it again to him.

He was a virgin when he married her at 26, while she had been wed before, in a shotgun marriage after she became pregnant at 16. “She insisted that she really wanted to do it for us. After the surgery in November 2004, we waited and waited while she healed up. It was all planned for a romantic Valentine’s weekend. I was nervous — we both were. It felt very strange — but it was also instant pleasure,” says Mr Yarborough. So did it hurt? “Oh yeah,” says Mrs. Yarborough, almost proudly. “And I bled a bit.”

She had originally gone to see the specialist who performed the operation because she developed a bladder problem after having two children. The female doctor, Dr. Troy Hailparn, explained that not only could her intermittent incontinence be solved but she could get a whole new lease on her sex life. “I had always been a woman who could have great orgasms and I never had a problem,” says Mrs. Yarborough. “But suddenly I was getting these gushes when we had sex and it was really embarrassing. I stopped even wanting to have sex.”

Dr. Hailparn claims that many doctors and gynaecologists are not good at dealing with post-childbirth incontinence, let alone wanting to do anything about a couple’s waning sex life, or discuss the value of virginity. She offered to use laser surgery to trim and tighten Mrs. Yarborough’s birth canal, simultaneously curing her leaky bladder and giving her a more youthful vagina — even tightening it to the specific proportions of her husband’s penis. “Now it’s like a glove and we feel like we are in our early twenties again,” says Mr Yarborough.

Dr. Hailparn’s clinic in an anonymous office building on the outskirts of San Antonio mixes soft furnishings and gynaecological models in the waiting room. She conducts the examinations at the clinic but performs the actual surgery in a rented operating theatre elsewhere. She is petite and very approachable in her floral skirt, cardigan and dangly earrings below wavy hair. When I go to talk to her, Dr. Hailparn immediately says how important it is for a woman to feel good about her body and her sexuality. She brings out multiple close-up “before and after” pictures of women who have had their vaginas tightened, lips trimmed or bladder de-leaked. “Now, let me find you a hymen,” she chirps, before skipping into her office to find a digital photograph of a neatly restored quasi-virginity. It is not for the weak-stomached.

She gave up her job as a hospital obstetrician and gynaecologist three years ago to train with Dr. Matlock and now runs a business that she calls The Laser Vaginal Rejuvenation Institute of San Antonio. Amid all the tightening and trimming, she has performed ten hymenoplasties in the past two years. She runs huge adverts on highway billboards and in local newspapers offering to make women “Feel Sexy Again ” and says she knows, as a 45-year-old mother, better than any male surgeon what a woman needs to regain both her urinary and sexual health after multiple childbirths.

News of the growth of hymenoplasties is sparking protest in cyberspace. “This is so sickening, so disturbing. How can women hate themselves this much?” asked one Elizabeth W on a chatboard. Weblogger “Bodhi Girl” pasted this comment from her boyfriend: “If you’re going to spend $5k, honey, get yourself some big jugs.”

It is the sheer indulgence that got to Dahlia, a New York blogger. “Children are going to bed hungry in this city. But wealthy women are having fake hymens for thousands of dollars to “add that extra spark” to their marriages, then have it undone in one night? I’m angry and sad. My brain explodes,” she writes. Dr. Leonore Tiefer, a New York sexologist, has a different concern — that women are allowing surgeons to dive in with intimate surgery that has not been officially researched or tested, either physically or psychologically.

“When it is ‘the new thing’ with very little data available, how do people know it is OK? No approval is needed, such as is required for a new drug. To do a novel surgery you just have to have the idea. We are now seeing people who had other “novelties” such as Botox and penis enlargement surgery coming in with irreversible damage,” she said.

However, Dr Bernard Stern, a Florida surgeon who offers the procedure, believes the worst damage would be to leave women unfulfilled. “I’ve got an e-mail here I just had in from a potential patient. ‘Dear Dr Stern, I’m 41 and I’ve had four children. Three years ago I got divorced and I just met someone new. I would like to come in for vaginal tightening, labiaplasty and hymenoplasty, please.’ I did a hymenoplasty on a British Muslim girl last week. I get 30 to 40 e-mails a night, three about the hymens. Vaginal tightening is becoming as common as the tummy tuck and it is something that should be more widely talked about.

“Sometimes their husband left them for someone younger, prettier and tighter,” he says.

“It is a Pandora’s Box,” New Jersey’s Dr Pelosi admits. “Younger women want to make themselves like the Playboy bunny. But for older women it is about regaining something long lost. Maybe putting value on something again in a society where sex is on the television all day long, it’s everywhere.”
For Jeanette Yarborough, it was also about savouring an experience she had really not enjoyed the first time round. “When you are older and with someone you truly love who treats you well, there is so much more intimacy,” she says. After her surgery, she enjoyed sex again so much that she almost had to put Louis on Viagra to keep up. He giggles, bashfully. At 40, she experienced something that few women do. Although I had heard of cheaper cosmetic surgery procedures in Thailand, I had no idea where to begin to look. We wound up with his number one choice and being that I was young and not needing to remove much fat were able to arrange a shorter stay.

In hindsight I am glad that they would not compromise on using only the best surgeon as the result is far more than just success or failure, but works on a scale of how much of a success or failure. The surgeon definitely knew what he was on about and I would recommend that anyone lets them make their best judgment about what will work for you. Originally I was not fussed with my fat removal from my lower abdomen but it is now probably the most pleasing result.

ThaiMed ensured I was met at the airport and taken to the 5 star hotel arranged for me as part of the overall price, the centrally located Grand Mecure. They provided me with a mobile phone, offered to show me around the city, drove me between all appointments and airport transfers and acted as a support network during my hospital stay and throughout the entire experience.

I have been informed that everyone's liposuction experience and recovery is different but within 1 month the bruising entirely and more than 2 months later I cannot give a higher recommendation to Mike and the ThaiMed team.

Thank you for the wonderful surgical experience organized by ThaiMed. I travelled alone, but from the moment I arrived in Bangkok I knew I was in excellent hands. A smiling young lady driver whisked me away to the hospital where I was quickly registered and escorted to a large comfortable private room. My vital signs were checked and I was left to rest.

Next morning my personal interpreter came to take me to all my prearranged appointments and consultation with a top plastic surgeon, followed by chest x-ray. E.C.G. and blood tests. My operation was then scheduled for 5p.m. the same afternoon.

First let me say that Mike and Richard are tremendous help with everything pertaining to the procedure or procedures that you need or want.

First I went to Bangkok Smiles Dentist to have my teeth whitened; the doctor found several cavities and filled them also. He did a wonderful job on my teeth. I even have a partial plate and after the procedure it did not fit right the Dentist did not even bat an eye he just went to work and made the plate fit again, actually better than it did before.

The second procedure I had done was a hair transplant. I went to the hospital in the morning and had my blood taken and an EKG to make sure I was up to surgery. Later that day I under went the first of two surgeries this was done at in the Hospital where the doctor was very meticulous.

He wanted everything perfect. First he harvested the donor hairs from the back of my head and sutured the area. I went to a very nice hospital room to rest while his staff painstakingly separated all 1350 grafts. Then I was taken back into the operating room.

Where he deadened my head then proceeded to implant each graft. This started at noon and lasted until 4 pm. He never stopped the room was filled with talking and laughing to break the monotony of the surgery. I thought this was wonderful, I was to receive 1200 grafts but he had the donors and he was there so he put in 1350. He is an excellent doctor and has an excellent staff.

My next procedure was Vaser Liposuction in four different areas. It to was done at the Hospital.

The doctor I had was very good he paid attention to ever detail. I spent the night in the only way I can describe it was a suite. The nurses took extremely good care of me all night long. I had three procedures done while I was in BKK, and I will say this about the people and hospitals I was in, I have very good insurance in America but I can tell you for certain when I need another procedure done I will go thru Thaimed and Mike to have it done. After my experience with Thai Doctors and nurses I have to say with out a doubt their attention to detail and wanting everything perfect has sold me. I am 50 years old and been in American hospitals many times through out my life but I have never been treated with such care and attention to every detail about every procedure. When or if I ever need anything else done I will go back to Thailand to see Mike, Richard and the absolutely wonderful doctors and nurses at the Hospital.

Vaginoplasty (vaginal rejuvenation) Female Cosmetic Plastic Surgery Thailand

Vaginoplasty (rejuvenation of the vagina) involves “tightening” and rebuilding of the vaginal muscles and the perineum. Childbirth, (especially multiple), aging, and genetic factors often cause the vaginal muscles to loosen, tear, and weaken. The diameter of the vagina gets bigger and there is a loss of feeling (and pleasure). Vaginoplasty returns the vagina to “pre-pregnancy” state and recreates sexual excitement and pleasure for the woman (and the man)! Some doctors use a laser for vaginoplasty and or labiaplasty surgery. You may hear varaitions of terms such as: labiaplasty or laser labiaplasty or vaginal rejuvenation and vaginoplasty or laser vaginoplasty or vaginal tightening. The difference is that some doctors are trained to use the laser for cosmetic vaginal surgery and reconstructive vaginal surgery and some use a scalpel.

When the vagina becomes stretched from aging and child birth vaginoplasty tightens and restores the vagina and supporting structures to a "pre-pregnancy" state, thereby re-establishing friction that increases sexual gratification for both women and men!

In vaginal relaxation, the muscles are relaxed and have poor tone, strength, and control. The internal and external diameters increase. The muscles of the perineum are weak and poorly supported. Under these circumstances, the vagina is no longer at its optimum physiological state. As a result, the sensual side of sexual gratification is diminished. Vaginoplasty can enhance sexual gratification for women who for whatever reason lack an overall optimum architectural integrity of the vagina.

No one wants to age or lose optimal function anywhere, and this includes the vaginal and vulvar structures.

After surgery, the patient is usually able to walk comfortably within a few days and may return to sexual activities within 4-6 weeks.

Most surgeons only perform hymenoplasty or hymen repair (hymen restoration surgery) at the request of someone who needs the surgery for ethnic, cultural, or religious reasons. Also, most surgeons will not perform hymenoplasty surgery in a woman who has given birth.

The hymen is a ring-like skin membrane that sits in the lower 1/3 of the vagina. It marks the spot that the vulvovaginal bulbs fuse with the Mullerian ducts from above then hollow out to form the vagina. Most often there is a 5 or 6 pointed star opening in the hymen after maturity. With first intercourse or by accident from falling or tampons, 2 or 3 areas tear in the hymen. Most often there is bleeding at the time of tearing.

The hymenoplasty procedure (to reconstruction the hymen), is after using local anesthesic for tissue dissection and to stop small blood vessels from bleeding, the areas which were torn are "denuded" meaning the upper layer of tissue is removed. This is so that they will grow back together when they are approximated with stitches. Then after they are denuded, the edges are brought back together to reform the star-shaped "ring" as it was prior to relations, accident etc. It is made small enough, so that when first sexual relations occur later on, it will "tear" again, there will be some pain, and there will be bleeding.

Selaput dara bisa saja sudah robek tetapi si gadis masih perawan, atau sebaliknya, selaput dara tidak robek padahal si gadis sudah tidak perawan. Hal ini bisa terjadi karena definisi "tidak perawan" adalah pernah melakukan hubungan seksual dengan lawan jenis, tidak peduli apakah selaput daranya robek atau tidak.

Hymen adalah lapisan mukosa yang mengelilingi atau menutupi sebagian dari muara vagina. Lapisan tersebut, seperti halnya mukosa vagina, juga mempunyai pembuluh darah dan pembuluh saraf. Oleh sebab itu, robekan pada hymen seringkali diikuti dengan perdarahan dan rasa nyeri.

Pada saat seorang gadis baru memasuki usia sekolah, selaput dara masih tipis dan bahkan hampir tembus cahaya. Oleh karena itu, pada usia ini, selaput dara lebih mudah mengalami robekan, terutama akibat olahraga seperti berkuda, bersepeda, atau senam.

Setelah pubertas, akibat pengaruh hormon estrogen, hymen akan sedikit menebal dan berwarna merah muda. Tapi perlu diingat bahwa ketebalan, bentuk, dan elastisitas hymen berbeda-beda pada setiap wanita.

Belakangan ini berkembang teknik operasi untuk mereparasi hymen yang terlanjur robek. Baik akibat kecelakaan maupun karena hubungan seksual. Teknik operasi tersebut disebut hymenoplasty.

Tujuan hymenoplasty adalah mengembalikan hymen seperti keadaan sebelum terjadinya robekan. Biasanya dilakukan karena alasan moral, budaya atau sosial.

Hymenoplasty termasuk operasi kecil. Biasanya dilakukan dengan bius lokal atau sedasi. Teknik operasinya ada dua macam, yaitu simple hymenoplasty dan alloplant.

Simple hymenoplasty dilakukan jika selaput dara hanya mengalami robekan dan masih ada yang tersisa. Pada bagian yang robek, dilakukan penjahitan, biasanya dengan benang yang dapat diserap, sehingga selaput dara kembali ke bentuknya semula.

Tetapi, jika selaput dara sudah rusak berat atau hilang sehingga tidak mungkin lagi dijahit, operasinya dengan teknik alloplant. Pada alloplant, dilakukan pemasangan selaput dara buatan.

Berikutnya timbul pertanyaan. Apakah seorang yang telah menjalani operasi hymenoplasty menjadi perawan kembali? Jawabannya adalah, jika kita merujuk kepada definisi "tidak perawan" di awal tulisan ini, hymenoplasty tidak mengembalikan keperawanan.

What Is Hymenoplasty?

There are two types of hymenoplasty - simple hymenoplasty – a repair of a torn hymen by sewing the remnants together, if the remnants are present. The other is a reconstruction of a new hymen from patient’s own tissues. Reconstructions of the hymen using alloplants or biomaterials that can be torn through is no longer done. Hymenoplasty is the restoration of the hymen to its original “virgin” state. It is referred to as hymen repair surgery, hymen restoration, hymen reconstruction or hymen tightening.

The hymen is a mucous membrance that partially covers the external opening of the vagina. It can have many shapes. It can look like a crescent with a curve border, it can have a straight border and cover half the opening of the vagina or three quarters, it can cover the whole opening and have many small perforations in it, it can also be in a form of a band going across the opening. Other shapes include a total covering with a star type opening or a trianglular shapes can also occur. Thus the size and shape of the hymen varies. Some females can be born without a hymen although it is extremely rare. Others can have a very thick hymen that must be opened by surgery. Often estrogen creams are applied right after birth to thin out a very thick hymen if it is noticed in the newborn. It must be noted that the hymen must have a perforation in it, to allow for any discharge or menstruation to pass through. Thus it is never left as a solid structure. Once the hymen is broken it does not heal spontaneously but must be reconstructed if one wants to have one.

Hymen repair requires the use of adjacent tissue to be sewn into the very same spot where the original hymen was. A Hymenoplasty is not necessary but usually requested either to fulfill ones wish to look like a virgin in a traditional wedding or on first encounters with a new mate. Hymenoplasty can be performed any time after the hymen has been ruptured.

A hymen can be torn during sexual intercourse, or as a result of horseback riding, tampon use, biking or other exercise or activities such as gymnastics. Many women seek to undergo a hymenoplasty for religious, ethnic or cultural reasons. An intact hymen is a symbol of virginity and sexual purity and innocence and is required in some traditional weddings.

In some cultures and religions, it is imperative that the woman be a virgin when she is married. Her hymen will literally determine her future prospects of marriage and could impact her family’s reputation. The woman, in that type of an atmosphere knows that her having an intact hymen will impact her social and economic future. Her husband can punish his new bride, or even abandon her if she lies about her virginity. If she is not a virgin at marriage then it is a sign of disrespect for her future husband and his family.

Typically a hymenoplasty can be performed on an outpatient basis with the patient under local anaesthesia with some sedation. The procedure lasts about one hour. Hymenoplasty heals well and one cannot tell that it is not the original hymen. The hymen heals without scars. Generally the patient can return to work the day after surgery and the hymen should be healed within six weeks. Sexual activity if undertaken after six weeks from the procedure will simulate breaking a hymen of a virgin. The first time intercourse will result in bleeding and/or discomfort and pain as the new hymen is torn again.

Hymenectomy should be done by an experienced surgeon whom you have researched as to his credentials. The hymen repair is a delicate and very personal surgery and you and your surgeon should discuss it fully before undergoing the procedure. You should also feel comfortable with your surgeon and discuss your reasons, fears and hopes as related to hymen reconstruction. Costs can range from a price as low as $2500 to as high as $5,000. Some women combine this type of surgery with other vaginal or labial surgery so that they can feel really good about themselves when they have recovered knowing that their sexuality has been determined by them.

Hymenoplasty is a sexist, misogynist farce, which is of NO benefit to women, and only exists because of the prevalence of myths about the hymen and virginity, and because some cultures still put a high value on the pain and discomfort of women as something good. Hymenoplasty -- the "restoration" of the hymen, a piece of tissue which women do not need, and which offers them no sexual benefits will cause a woman to have pain and bleeding afterwards when intercourse occurs. I am not going to go into the way the procedure is done because a) it sickens me and b) this is outside our scope.

Many women who have not had vaginal intercourse do not have fully intact hymens: the hymen wears away, over time, due to a number of factors, not merely intercourse. For those who have fully intact hymens -- a rarity -- at first intercourse, intercourse is painful if not impossible. So, if you have the idea that all women who have not had vaginal intercourse have a fully intact hymen, you're quite mistaken.

I have a question Heather about that if you don't mind. Hope its okay if I post it there. I know this isn't a subject you particularly like, me neither, but I'm a little curious about something.

I agree 100 % with you but I was wondering if hymenoplasty restore the entire hymen, if it gives a woman an intact hymen or a partial hymen. To me, the answer would be an intact hymen since most people often associate virginity with intact hymen although it doesn't really exist except for a few exceptions who will need a small surgery.

But how can a women menstruate with that restored hymen (if it is intact) if there's no hole in it ? I mean unless she's getting her hymenoplasty right before she has sex or few weeks before that, the hymen seems to occasionnate a problem per menstrual flow no ? Or it will break and not stay intact? It just doesn't make sense to me.

It's not pleasurable at all for the women.

The women who do it either due because of cultural pressures in certain areas to have to bleed at first intercourse/marriage (and some of those cultural pressures literally can endanger their lives if it is thought they are not virgins, no matter how misinformed it is to base that on bleeding), or because someone's husband/pboyfriend fetishizes antiquated ideas about hymens and virginity.

So, no, for thw women it is not exciting. But some men fetishize this, and to them -- no matter that it causes women pain and discomfort -- it is exciting.

Which is why I'd prefer not to even talk about this, because like anything else where women's suffering is a turn-on for someone, as a woman, as an advocate for women, it tends to result in making me terribly sad and angry.

The attitude of treating woman as an object of sexual pleasure or as object in other means may result in various means of abuse. However, I think there are other reasons why a lot of people fancy the virginity concept and obsess about hymen. It is also not necessarily because of the feeling to own woman, in fact most guys ( at least in Asia ) don’t really understand why it is hard for them to accept woman that is no longer virgin. They call the non-virgin woman “been used” or “secondhand”, deep inside their mind they feel non-virgin woman is not as valuable as before when she was still virgin. I think this kind of mind is flawed, in any sexual activities including sexual intercourse, when it is enjoyable by the two without any pressure, both the woman and the man, no one is being used. Woman is passively active, and man is actively passive, none of them really passive or active. Hence, to be fair, non-virgin man also eligible to be called “been used” or “secondhand”

They are guys that treat woman as object, and they have a great satisfaction when having sexual intercourse with virgin woman and after that they dump the woman, they have this feeling of marking woman (something very similar with ownership), and they take proud of breaking a woman’s hymen. This feeling of marking woman also doesn’t make sense. I used to understand that kind of feelings, but since I have thought out this matter carefully, asking a lot of why, suddenly now I lost that feelings, I don’t understand it, weird. Maybe a lot of guys fancy virginity concept because they don’t really think and analyze their feelings, instead they let themselves become the victims of social conditioning. Hymen is really nothing to be adored of, if hymen will give so much pain to a woman, it is even better to remove the hymen surgically when she was born. When a female reach adult age, it is natural to have a sexual desire, and to have a sexual intercourse is her basic right, just like eating or peeing, and hymen could be just like an “obstacle” in a sexual intercourse for her(while not necessarily so). Why a man that have slept with a lot woman is highly respected while a woman that have slept with a lot of man is highly condemned while it is the exactly the same equal basic right for man and woman to fulfill their sexual drive?

And the media; newspaper, television, magazines, etc (at least again here in Asia), boosting up the virginity concept, influencing a lot of people, males and females to think that virginity (hymen related or not) is a super sacred matters. The act of making love itself is sacred but not the organs that we have. A lot of females here in Asia also “think” too much of their virginity and make sacred their hymen, a lot of women here once they are not virgin anymore, they tend to be too clingy and attached toward their partner, although they actually know that their partner is very abusive and no good to them, this situation is quite pathetic I guess.
